Feedback from local PIES users

If you are a cancer patient, carer or professional living or working within the districts of Barnsley, Bassetlaw, Chesterfield, Doncaster, Rotherham or Sheffield, you may have already completed and returned a paper questionnaire. If so, you need not fill in this online version which has essentially the same content. If you are a local user who has not returned a paper questionnaire, we would be very grateful if you use the online questionnaire below to give us your views. If you have already completed the online or paper versions but now want to send additional comments, or you simply prefer not to go through the questionnaire just now, you may use the Short Feedback Form instead.
